How Do I.... Register With The Practice?New patients who live within our defined practice boundary of Simmondley as shown on the back cover of this booklet (if in doubt please enquire at reception) may register with the practice. You will need to call in to the surgery to complete a registration form; it would be useful if you have your NHS number available. If you move outside the practice area after you have registered, you may be asked to find a doctor closer to your new address. How Do I.... Change My Details?You may write to us, telephone or call in person to notify us of any change of details. We do not require any proof of the above except in the case of those children who have had their name changed through a legal process. |
