The Glossop Health Team (Glossop Primary Care Network)

Glossop Health Team works for and on behalf of Simmondley Medical Centre, Lambgates Health Centre, Howard Medical Centre, Cottage Lane Surgery and Manor House. You may be offered an appointment by a member of the team :

Social Prescribing Link Worker

Social prescribing is when health professionals refer patients to support in the community, in order to improve their health and wellbeing. Our link workers are employed by The Bureau in Glossop as well as in the local practices. They offer support either by telephone or face-to-face at the surgery.

Health & Wellbeing Coach

Health coaching is a service offered to help create lasting change to improve a person’s wellbeing and overall health.

Musculoskeletal (MSK) First Contact Practitioners

Patients now have the opportunity to see a musculoskeletal (MSK) practitioner as their first point of contact for new problems, knee pain, shoulder pain, back pain etc. Practitioners can come from a range of disciplines including physiotherapy, osteopathy and podiatry; designed to deliver a streamlined, person-centred service and support the growing demands on general practice and secondary care.

Mental Health Practitioner (MHP)

MHPs work with our patients to support engagement with appropriate services and treatment which may include onward referral to secondary mental health services or sign-posting to other mental health services, voluntary services and support groups.

Paramedics

Our paramedics provide face to face consultations for acute (new) medical concerns. They also provide acute home visits for vulnerable, elderly and unwell patients.

Citizens Advice

An advisor holds a weekly session at the surgery.
